2011 Annual
Membership Documents
The following items need to be completed and delivered to the AMKA
Membership
Registrar before the member driver may participate in any 2011 AMKA Events
(click to obtain applicable documents):
1. AMKA Membership Application Form
2. Payment of annual membership fee
(including for any associate member)
3.
Medical self-declaration of the driver
4. Copy of birth certificate of the driver
5. Proof that driver has completed AMKA orientation/school
If the driver member is under 19 years of age as of May 1,
2011, the following
additional items are required:
6. ASN Canada FIA Annual Parental Consent Waiver
7. ASN Canada FIA Consent & Release by Parent or Guardian
If the driver member is over 50 years
of age as of May 1, 2011, the following
additional item is required:
8. Physical Exam Form (to be completed every 2 years)
Upon
receipt of properly completed documents, AMKA will issue each driver and
associate member
a unique membership number and a Kart Driver Competition Licence to the driver.
If a new race kart driver, the membership applicant must complete an AMKA
orientation/school
(call or email AMKA Directors for details)
Race Day Documents
1. Technical
Passport (tech sheet) to be completed for the entered kart
on race day
2. ASN Canada FIA Minor Participant Waiver (if driver is under 19)
3. Payment of race day entry fee
4. Be able to present his or her AMKA Kart Driver License if requested
